bradycardia, lethargy, hypoventilation, decreased shivering and atrial arrhythmias
mild hypothermia
32-35 C or 90-95F
tachycardia, tachypnea
ataxia, dysarthria, increased shivering
severe hypothermia
<28 C (82F)
coma, cardiovascular collapse, ventricular arrhythmias
Treatment of hypothermia in general
warm 42C (107F) crystalloid for hypotension
endotrachial intubation for people who are comatose
Rewarming techniques are based on severity of hypothermia
Mild hypothermia treatment:
passive external warming (remove wet clothing and cover with blankets)
moderate hypothermia treatment
active external warming (warm blankets, heating pads, warm baths)
severe hypothermia treatment
active internal rewarming (warmed pleural or peritoneal irrigation, warmed humidified oxygen)
why is there sometimes an initial drop in core temperature after starting IV warmed fluids for pts who have hypothermia?
because of return of cold blood from cold extremities.
if severe hypothermia for moderate hypothermia that doesn’t respond to initial measures what to do next?
warm pleural or peritoneal irrigation, cardiopulmonary bypass or venoartierial extracorporeal membrane oxygenation.
This can rewarm faster.
treatment of cardiac arrhythmia associated with hypothermia
cardiac arrhythmias like afib will spontaneously convert and correct with rewarming.
