Resp Flashcards
Level of superior scapular angle
T2
Medial spine of scapula
T3
Level of inferior scapula angle
T7
Suprasternal notch level
T2/3
Superior and inferior levels of manubrium
T2/3
T4/5
Sternal body levels
T5-T8
Level of xiphisternum
T8/9
Transpyloric plane level
L1
Sub coastal plane level
L2
Why do you do asymmetry on posterior side
Because gives idea of lower lobes
Anterior side only for middle+upper lobes
What to ask patient when percussing back?
To cross arms
Anterior sites for lung apex
1
Anterior sites for right upper lobe
2
Sites of left upper lobe
2,3,5
Sites for middle lobe percussion and auscultation
3,5
Sites for lower lobe
4
Surface markings of right upper lobe
Ribs 1-4
Surface markings of middle right lobe
Ribs 4-6
Surface markings right lower lobe
ribs 5-7 laterally
Surface marking left upper lobe
ribs 1-6 medially
Surface markings left lower lobe
ribs 5-7 laterally
Inferior margin of lower visceral pleura
Medially 6th costal, 6th rib MCL, 8th rib MAL and scapular line 10th rib
Reasons for chest drain
Pneumothorax, haemothorax and pleural effusion
Safety triangle for triangle
Superiorly axilla and inferiorly 5th ICS
Lateral border of pec major and anterior border of lat Dorsi
Where to place needle for chest drain
Upper border of rib
Direction of medially upwards
Best if anterior to MAL in order to avoid long thoracic nerve
Inferior border of right lung parietal pleura
Starts at transverse process of T1 then goes around root of neck. Projects above 1st costal cartilage to inch above medial 1/3 of clavicle. Runs down sternum to 6th costal cartilage and then reflects onto diaphragm above costal margin, goes around thoracic wall. Inferior boundaries are medially 6th costal cartilage, 8th rib MCL, 10th rib MAL, 12th rib and transverse process of L1 vertebral
Inferior borders of left lung parietal pleura
Starts at transverse process of T1 then goes around root of neck. Projects to left sternoclavicular joint an inch above medial 1/3 of clavicle. Runs down sternum to 4th costal cartilage and deviates to left because of cardiac notch. Inferior boundaries are, 8th rib MCL, 10th rib MAL, 12th rib and transverse process of L1 vertebral
Horizontal fissure
Runs from 4th CC to meet onlique fissure in MAL
Oblique fissure
Runs from 6th CC to T3 posteriorly
