Instead of using LMWH until the diagnosis is confirmed, NICE now advocate using a DOAC once a diagnosis is suspected, with this continued if the diagnosis is confirmed
Staphylococcus aureus pneumonia
blood-streaked (or rusty) sputum, pleuritic chest pain, cavitating lesion X-ray
often proceeded by influenza!!1

- then maybe theophylline
