Testicle rotates and strangulates spermatic cord and vascular supply
Acute onset excruciating unilateral pain +/- N/V
Use Testicular US to differentiate between torsion and epididymitis

Hydrocele/Varicocele

Answer 3

Hydrocele - communicating (patent tunica vag. with fluid) non-com (obliterated tunica vag. with fluid trapped in scrotum)

• most resolve spontaneously in 12mo (observe)
• persistent might require surgical closure to prevent novel herniation and incarceration

Varicocele - abnormally dilated pampiniform venous plexus, MC on the left, “bag of worms”
