Post Op Atelectasis
A
Increased pharyngeal secretions, airway edema, postoperative pain leading to shallow breathing all increase risk of atelectasis
Can lead to V/Q mismatch which typically presents postoperative night two
collapsed alveoli–>hypoxemia–>hypervent–>low co2–>high pH
(Same ABG seen with acute PE)
PreOp prevention: 8wk smoking cessation, tx of pulm disx
PostOp prevention: breathing exercise, incentive spirometry, epidural pain control vs systemic opiates
