Description
What Happened? What was the critical event?
Feelings
What were your feelings? How did you feel at the time, how do you feel now?
Evaluation
What was good and bad about the experience? Both for you and others involved. What was positive and negative?
Analysis
What sense can you make of it? What can you learn from the experience?
Conclusion
What could you have done differently, looking back now?
Action Plan
If it happens again, what would you do? IF a similar set of circumstances happened again, what would you do?

Description
The description is always first; Describe the experience. What factors contributed? What was the context? What was special about the experience?
Reflection
Why did you act the way you did? What were the consequences on yourself and others? What are you r feelings and those of others? How do you know the feelings of others?
Influencing factors
What internal and external factors influenced the choices you made? What knowledge influenced the choices? What other choices were available? What do you think the outcome would have been for the other available choices?
Learning
How has this experience changed your knowledge base? how will it affect future choices? How has it changed your ethical knowledge? How has it changed your knowledge about yourself? How has it changed the way you view other people or the world around you? Can you make any sense out of this experience? Could you have done anything else? What would you do differently?

Recount the event; what were you goals and expectations at the time? Don’t talk about what you felt.
Feelings
After the event, what did you feel about yourself and others? What causes these feelings? Take ownership.
