Rapidly Progressive Glomerulonephritis Flashcards
How many types of rapidly progressive glomerulonephritis are there?
Three
What is type 1 RPGN?
Anti GBM antibodies
- Goodpastures syndrome
What is type 2 RPGN?
Immune complexes
- Post strep
- LUPUS
- IgA nephropathy
What is type 3 RPGN?
ANCA
- No antiGBM or immune complexes
- cANCA
- pANCA
What is cANCA?
Cytoplasmic anti neutrophil cytoplasmic antigen
- Granulomatosis with polyangitis
What is pANCA?
Perinuclear anti neutrophil cytoplasmic antigen
- Churg Strauss syndrome
What is the pathology of RPGN?
Break down of the GBM causing protein and blood to move out
- Macrophages and monocytes move out
- These proliferate in bowman’s space
Symptoms?
AKI
Tiredness
Anorexia
NEPHROTIC SYNDROME
Signs?
Haematuria
Proteinuria
Tests?
Renal biopsy
What should be done with the renal biopsy?
Histology
Electronmicroscopy
Immunofluorescence
What will be seen on histology?
Crescenteric shape of Bowman’s capsule
What will be seen on immunofluorescence in type 1?
Linear spread of antiGBM on collagen
What will be seen on immunofluorescence in type 2?
Granular immunocomplexes
What will be seen on immunofluorescence in type 3?
Nout
