What is the pathology of focal segmental glomerulonephritis?
Scar tissue forming in the glomeruli
Podocytes damaged -
- Plasma protein and lipid enters bowmans space
- Build up and hyalinose
What syndrome does focal segmental cause?
NEPHROTIC SYNDROME
What is nephrotic syndrome?
> 3.5g/day of proteinuria
Hypoalbinaemia
Hyperlipidaemia
What does hypoalbuminaemia cause?
Oedema
What does hyperlipideamia cause in nephrotic syndrome?
Lipiduria
Investigations?
Renal biopsy
Bloods
What tests should be done with the renal biopsy?
Histology
Electronmicroscopy
Immunofluorescence is not relavent in focal segmental
Findings on histology?
Hyalinosis
Sclerosis
Findings on electronmicroscopy?
Loss of podocyte foot processes
Bloodwork investigations?
Creatinine raised
Albumin lowered
Hb
Treatment?
Loop diuretic
LMW heparin
Prednisolone
If the prednisolone works what should happen to the other treatments?
Stop them immediately
