Post Infective Glomerulonephritis Flashcards
1
Q
Aetiology?
A
Streptococcal infection of skin/throught
2
Q
What is special about the type of bacterium involved?
A
Needs to be Group A beta haemolytic
These carry the M protein virulence factor
- Nephritogenic
3
Q
Pathology?
A
Type 3 hypersensitivity
- Immune complexes deposited in GBM
- C3 compliment laid down
- Cytokines and proteases damage GBM so large molecules filtered through
4
Q
Symptoms of post infective?
A
Nephritic state
5
Q
What is the nephritic state?
A
- Haematuria
- Proteinuria
- Oliguria
6
Q
Signs?
A
Periorbital and peripheral oedema
7
Q
Tests?
A
Dipstick first
Renal biopsy
8
Q
What should be done with the renal biopsy?
A
Histology
Electronmicroscopy
Immunofluorescence
9
Q
Histological findings?
A
Enlarged hypercellular glomeruli
10
Q
Electronmicroscopy findings?
A
Subepithelial deposits
11
Q
Immunofluorescence findings?
A
Starry sky pattern
- Granular GBM
12
Q
Treatment?
A
Should resolve on its own
- Monitor renal function
- Stop nephrotoxic drugs
