Generalized response: reacts inconsistently and non-purposefully, responses are limited and same regardless of stimuli

Localized response: reacts specifically but inconsistently, responses directly related to stimuli. May follow simple commands in an inconsistent delayed manner

Confused Agitated: heightened sense of activity, behavior is bizarre and non-purposeful relative to immediate environment. Does not discriminate among persons or objects; unable to cooperate with treatment efforts. Verbalizations incoherent or inappropriate. Pt lacks short and long term recall

Confused Inappropriate: Responds to simple commands consistently. Has gross attention to environment, but is highly distractable and lacks ability to focus. Verbalization is inappropriate. Inappropriate use of objects. Can perform previously learned tasks with structure, but unable to learn new info

Confused Appropriate: goal-directed behavior, but dependent on external input. Follows directions consistently and shows carryover. Responds incorrectly due to memory problems, but are appropriate for situation. Past memories have more detail and depth than recent memories

Automatic Appropriate: Appropriate and oriented in hospital and home setting. Goes through daily routine automatically, but robot like. Minimal to no confusion and shallow recall of activities. Carryover for learning but at a decreased rate. Initiate social or recreational activity with structure. Judgment impaired
VIII
Purposeful Appropriate: Recall and integrate past events and aware of and responsive to environment. Shows carryover for new learning and needs no supervision. Decreased ability relative to premorbid activities, abstract reasoning, tolerance for stress, and judgment in emergencies or unusual circumstances
