Generalized response: inconsistent response to stimuli
Rancho 3
Localized response: Pt reacts specifically but inconsistently to stimuli. may follow simple commands such as closing eyes/squeezing hand in an inconsistent/. delayed manner.
Rancho 4
Confused-Agitated: heightened state of activity. bizarre and non-purposeful behavior, unable to cooperate with treatment efforts, pt lacks short and long term recall.

Confused-Inappropriate: respond to simple commands fairly consistently. highly distractable,
Rancho 6
Confused-Appropriate: Pt shows goal directed behavior. follows simple directions and shows carryover of relearned tasks such as self-care.
Rancho 7
Automatic-Appropriate: pt is appropriate and oriented to hospital or home setting. goes through daily routine automatically but robot like, min to no confusion and shallow recall. carryover of new learning, can initiate social activities with structured help. impaired judgment.

Purposeful-Appropriate: recall and integrate past and recent events, still may have impaired judgment, abstract reasoning with emergencies/ unusual circumstances
