when you have pyloric stenosis, the pyloris is enlarged (that’s why you can palpate it) and it’s so big, that it made the opening in the center of that sphincter teeny tiny, so now, the stomach is working hard to push fluid through that tiny little opening. this causes pressure to build up in the stomach (that’s why they have abdominal distention) and that’s also why they have projectile vomiting.
