PYLORIC STENOSIS Flashcards
A condition characterized by the narrowing of the pyloric
canal due to hypertrophy of the pylorus muscles.
Pyloric Stenosis
causes
idiopathic, pissibly familial
occurs how many weeks
2 weeks onwards
Symptoms of Pyloric Stenosis
- Projectile, non-bilious vomiting
- hunger and irritability
- olive-shaped mass in the epigastrium
- Dehydration and malnutrition
- Distended upper ABD
TREATMENT
pyloromyotomy
An incision through the muscle fibers of the pylorus that may be performed by laparoscopy
Pyloromyotomy
DX. TEST
- ABG- meta-alka
- Electrolyte decrease[Na, K, Cl]
- ABD ultrasound [fx and narrowing can be seen]
- x-ray [pic only]
Projective vomiting positioning
3-4ft. = SIDE
4ft. = PRONE
1ft. = SUPINE
s/sx
- PROJECTILE VOMITING
- PERISTALTIC WAVE L-R
- PALPABLE OLIVE SHAPED MASS
- FAILURE TO THRIVE
- INCREASE ICP
Increase ICP
Early:
* Restlessness and confusion
* Headache and vomiting
* Projectile vomiting
Increase ICP - VS
- Increasing Temp
- increasingBP
- decreasing PR
- decreasing RR
patho
hypertrophy of pyloric sphincter > narrowing of pylorus muscle> obstruction > increase pressure >
1. projectile vomiting
2. increase peristaltic movement [left to right]
