P: prior hospitalization within past 3 mo
What antibiotics should you use in the follow CAP case?:
outpatient, previously healthy, no risk for drug resistance
Macrolides (SOR A) or Doxycycline (SOR B)
What antibiotics should you use in the follow CAP case?:
Outpatient with comorbidities or recent antibiotics
Respiratory fluroquinolone (SOR A): moxi, gemi, levo
Or
Beta-lactam + macrolide (SOR A): amox/clav
Indications for PCV13 vaccine
Adults > 19 with CSF leaks, cochlear implants, functional asplenia, sickle cell, immunosuppression
All adults > 65 who have not previously received a dose of PCV13 need one dose
Indications for PPSV23 vaccine
single dose at age > 65 years
single dose:
- cardiac, liver, diabetes, lung disease, smoker, cochlear implants or CSF leak, resident of chronic care institution
2 doses 5 years apart:
- CKD
- asplenia, sickle cell disease
- immunocompromised
How to give both pneumococcal vaccines in Age 2-18
after last dose of PCV13 give one dose of PPSV23
How to give both pneumococcal vaccines in age 19-64 at high risk
give PCV13 first followed by PPSV23 at least 8 weeks later
How to give both pneumococcal vaccines in age > 65 yo
Give PCV 13 first followed. by PPSV23 1 year later
When is a tuberculin skin test positive at > 5 mm
- HIV seropositive
- recent TB contact
- CXR shows prior inactive TB
- immunosuppressed patients
