A

Adapted starvation

  • reduced carb intake
  • reduced insulin secretion

Main sources of energy are fat and protein

  • reduced intracellualr phosphate
  • extracellular phosphate may be normal

Low energy expenditure

Atrophied gut, heart, muscles

Low micronutrient reserves

A

Refeeding with carbs
Start slow
Correct fluid deletion
Thiamine at least 30 mins before feeding starts
Feed at 5/10 kcal/kg over 24 hours
Gradual increase to requirement over 24 hours
