The cause of inflammation and pain in chronic prostatitis
may be initially triggered by an infection, with inflammation persisting after the infection has resolved.
presentation - chronic prostatitis
3+ months of
- pelvic pain : perineum, testicles, sctotum, penis, suprapubic, groin areas
- LUTS
- sexual dysfunction : ED, haematospermia
- pain with bowel movements
- tender + enlarged prostate on DRE
presentation - acute bacterial prostatitis

- painful bowel movements
- tender and enlarged prostate on DRE
- systemic infective symptoms: fever, myalgia, fatigue, sepsis

Mx of acute bacterial prostatitis
- hospital admission if unwell/septic
- oral antibiotics
- analgesia
- laxatives
Mx of acute bacterial prostatitis - antibiotic choice
ciprofloxacin
ofloxacin
or
trimethoprom
Mx of chronic prostatitis
- alpha blockers
- analgesia
- psychological treatment
- antibx
- laxatives
Mx of chronic prostatitis - Antibx prescribing
if <6mo of symptoms or Hx of infection
trimethroprim or doxycycline for 4-6wks

sepsis
prostate abscess
acute urinary retention
chronic prostatitis
antibiotic choice
14days of quinolone antibiotic
