aromatic amines - dye and rubber
shistosomiasis - squamous cell carcinoma
types
transitional cell carcinoma (90%) squamous cell carcinoma (5%) adenocaricnoma (2%) sarcoma small cell carcinoma
main presenting symptom
painless haematuria
2 wk wait referral - when for >45yrs
unexplained visible haematuria
either w/o UTI or persisting after UTI Rx
2 wk wait referral - when for >60yrs
microscopic haematuris
plus either of
- dysuria
- raised WCC on FBC
diagnostic Ix
cystoscopy
can be rigid or flexible
local or general anaesthetic
non-invasive bladder cancer
not invading muscle layer of bladder
muscle invading bladder cacer
invades muscle and beyond
Tis/carcinoma in situ
cancer cells only affect urothelium + are flat
Ta
cancer only affects urothelium and projecting into bladder
T1
cancer invading connective tissue layer beyong urothelium
but not muscle layer
invasive bladder cancer- staging
included T2-4 and any lymph node or metastatic spread
Mx options
TURBT
intravesical chemotherapy
Intravesicle BCG
radial cystectomy
TURBT
transurethral resection of bladder tumour
can be used for non-invasive bladder cancer
