Why does anemia occurs in PMF if extramedullary hematopoiesis is occuring?
Extramedullary hematopoiesis is also disordered so that’s why anemia occurs
What is the pathogenesis of PMF?
Inappropriate release of fibrogenic factors from platelets leads to fibrosis
What are the fibrogenic factors released by platelets in MPD?
PDGF, TGF-B
What are the mutations involved in PMF?
JAK2, MPL, calreticulin

What are the clinical features of PMF?

Hypocellularity
Fibrosis
Megakaryocytes with cloud like nucleus
Hematopoietic elements move to dilated sinusoids
What happens in very late stage of PMF in bone marrow?
osteosclerosis
What are the changes in spleen in PMF?
Color changed (red to gray)
Size changed (4000 gm)
Subcapsular infarcts occur
EMH move from sinusoids to cords
