Dopamine (Intropin)

MOA:
Class: Sympathomimetic
MOA: Dose dependent, low doses act on dopaminergic receptors to cause renal, mesenteric, and cerebral vascular dilation, moderate dose works on beta1-andrenergic receptors enhancing myocardial contractility and increasing CO & BP, high dose acts on alpha1-andrenergic receptors to cause peripheral arterial and venous constriction

Indications: Hemodynamically significant hypotension in the absence of hypovolemia
Side Effects: Tachydysrhythmias, hypertension

Considerations:
Contraindications: Tachydysrhythmias; V-fib; pheochromocytoma
Considerations: Consider need to correct hypovolemia before dopamine infusion; use large vein to avoid injury from extravasation; use infusion pump when possible for more accurate dosing; monitor patient for signs of compromised circulation

Low, renal dose - 1-5 mcg/kg/min IV titration for desired response
Moderate, cardiac dose - 5-10 mcg/kg/min IV titration for desired response
High, vasopressor dose - 10-20 mcg/kg/min IV titrate for desired response
Dose Pediatric: 2-20mcg/kg/min IV, titrate for desired response
Onset: 2-4 minutes
Duration: 10-15 minutes
