ASA I
normal healthy patient
healthy, nonsmoking, no or minimal alcohol use
ASA II
patient with mild systemic disease
w/o substantive functional limitations
current smoker social alcohol drinker pregnancy obesity well controlled DM, HPN, mild lung disease
ASA III
severe systemic disease
substantive functional limitations
one or more moderate to severe diseases
poorly controlled DM or HPN COPD morbid obesity active hepatitis, alcohol dependence or abuse implanted pacemaker ESRD premature infant PCA < 60 wks history (> 3 mos) of MI, CVA, TIA, or CAD and stents
ASA IV
patient w/ severe systemic disease that is a constant threat to life
recent (< 3 mos) MI, CVA, TIA, CAD or stents
ongoing ischemia or severe valve dysfunction
severe reduction of EF
sepsis
DIC
ESRD not undergoing regularly scheduled dialysis
ASA V
moribund patient who is not expected to survive w/o the operation
ruptured abdominal or thoracic aneurysm
massive trauma
intracranial bleed w/ mass effect
ischemic bowel in the face of significant cardiac pathology or multiple organ or system dysfunction
ASA VI
brain dead patient whose organs are being removed for donor responses
