What happens to CO during pregnancy?
CO is increased especially in the L sidelying position. In this position, the uterus puts the lease pressure on the aorta. So, you want to recommend they lay in L sidelying.
-There is a decline in venous return resulting in increased CO which may lead to symptomatic supine hypotensive syndrome. Left sidelying takes this away
What happens to HR during pregnancy?
Resting HR usually increases 10-20 beats by full term and returns to normal within 6 weeks post pregnancy.
What happens to BP during pregnancy?
-Decreases early in the first trimester. The greater decrease occurs in diastolic
-If BP increases significantly the mother may be experiencing preeclampsia, MEDICAL EMERGENCY ***
What happens to TV during pregnancy?
Unchanged or slightly decreased
What happens to O2 consumption during pregnancy?
There is a 15-20% increase in oxygen consumption with a natural state of HYPERVENTILATION T/o pregnancy
What happens to breathing during workouts when pregnant?
Dyspnea is present with mild exercise as early as 20 weeks into pregnancy 2’ increased work of breathing from hyperventilation
What are the MSK changes during pregnancy?
-End of pregnancy stretches ABD muscles to their limit
-Pelvic floor drops up to 2.5 cm and may be stretched or incised during birth process which can lead to stress incontinence
What are the effects of aerobic exercise during pregnancy?
-Women’s respiratory rate does not increase proportionately with moderate and severe exercise
-The maternal hematocrit level during pregnancy is lowered and rises up to 10% within 15 minutes of vigorous exercise
-Compression of the inferior vena cava by uterus can occur after the fourth month of pregnancy
-Hypoglycemia occurs more often during pregnancy. Adequate carbohydrate intake is vital for exercising
-Core temp decreases during exercise. By regulating core temp more efficiently, the thermal stress to the fetus is reduced
What are contraindications to exercise during pregnancy?
-Incompetent cervix. Early dilation of cervix before the pregnancy is full term
-Placenta previa. Placentia is located on the uterus in a position where it may detach before the baby is born
-Rupture of membranes. Loss of amniotic fluid prior to the onset of labor
-Premature labor (prior to week 37)
-Maternal heart disease
-Maternal diabetes or HTN
What is diastesis recti?
Separation of the rectus ABD in the midline at the línea alba
Check for this before initiating abdominal exercise
-Test: Have pt hooklying and raise head/ shoulders to reach toward knees. PT places fingers horizontally across the midline of ABD and umbilicus. Measured by the number of fingers that can be placed between the rectus muscle bellies
Which exercises should not be performed if a woman has diastasis recti?
-Head lift, head lift w/ pelvic tilt, and pelvic floor (kegel) exercises CAN BE USED if the separation is less than or = to 2 cm or 2 finger widths
-Do not perform: double leg lifts, curl ups, leg lowering, and quadruped positioning until fully resolved
