- Vitals (BP, temp, pulse, RR)
- lung sounds
- BUBBLE-HE
subjective PP bleeding measurement
- scant (<2.5cm)
- light (<10cm)
- moderate (>10cm)
- heavy (pad saturated within 2hrs)
always ask how long pad has been in place
objective PP bleeding measurement
- H&H (weighing clots and other items that may be soaked with blood)
quantification of blood loss: weigh dry pad -> write number on back -> weigh wet pad ->subtract number on back
gentle fundal massage
maintain/restore uterine tone
palpating the fundus
- place upper hand above the fundus
- dip lower hand right above the symphysis pubis
kegel exercises
- strengthens the decrease in muscle tone from stretched and torn pelvic tissue
- reduces urinary incontinence
(start and stopping a urine stream)
Rh immune globulin
RhoGAM
- given to Rh neg mother at 28weeks gestation
- again within 72hrs after birth if Rh+ baby
- usual dose 300mcg IM
- made from human plasma (Jehovah’s Witness)
Rubella vaccine
MMR
- SQ prior to discharge
- prevent future rubella infection during pregnancy
- live vaccine
- may continue to breastfeed
varicella vaccine
chickenpox vaccine
- SQ prior to discharge
- live vaccine
- 2nd dose after 4-8weeks
Tdap vaccine
tetanus, diphtheria, acellular pertussis
- IM
- decrease infant exposure to pertussis (whooping cough)
- recommended by CDC in every pregnancy
PP health promotion
Drink plenty of fluids
Eat a well-balanced diet.
- Increase calories if breastfeeding
Do your Kegel exercises
Rest when the baby rests
Start with simple exercises.
- Abdominal exercises shouldn’t be started until 4-6 weeks post delivery for Cesarean clients
No tub baths for 6-8 weeks – prevent bacteria in vaginal area
Keep perineal area clean & dry, as well as c/s incision area
