early - less than 4 days , foul smell, clostridium
late - 7-10 days
what are the different stages of decubitus ulcers ?
stage 1 - non blanching erythema
2- involves the epidermis and dermis
3- reaches the subcutaneous tissue but not the fascia
4- reaches the fascia
when do we suspect wound dehiscence ?
1- post abdominal surgery
2- may feel a popping sound after coughing or straining
what is the management of wound dehiscence ?
urgent surgical intervention sometimes treated with VAC
what are the risk factors for wound dehiscence ?
diabetes
poor nutrition
Renal failure
what is paralytic ileus ?
following laparotomy , gastrointestinal motility decreases
what is the order in which the function of the bowel returns to normal ?
small bowel
then large bowel
then stomach
where are drains placed ?
near the incision site but away from the wound
what can drains aid in the diagnosis of ?
surgical anatomosis
what type off drain is used after opening of the common bile duct ?
T tube
what are the diagnostic indications for the use of a ryle tube ?
1- For gastric function tests-to assess free acid and total acid
2- Hollander’s test for completion of vagotomy
3- To diagnose trachea-oesophageal fistula
what are the therapeutic indications for the use of ryle tubes ?
1- In acute abdominal conditions like peritonitis/obstruction
2- In abdominal trauma
3- After abdominal surgeries
4- In pyloric stenosis
5- In upper Gastrointestinal bleeding
6- In paralytic ileus
7- For feeding purpose in conditions like comatose patients, faciomaxillary
8- Injuries, major head and neck surgeries
when are skin sutures or clips removed post op ?
6-10 days post op
when can the patient return to work after surgery ?
