Post Op Knee Rehab Flashcards
3 Systems of Function
- Articular
- Muscular
- Neural
Articular System
- joint motion
- joint range
- hypermobility vs hypomobility
Muscular System
- length/strength
- strength vs stability
- facilitation/inhibition
Instability
uncontrolled motion at passive end range
Neurological
- Neural length
- sensitivity
DAY 1
-work on full extension
Phase I Rehab
- ROM
- Pn
- swelling control
- restore muscular stability
- -Stability vs strength
Phase II Rehab
- strength
- type I fibers
- Phasic muscles
Phase III Rehab
-function
Meniscal Repair ROM Limitations
90* (4-6 weeks)
Pay Attention to these:
- uncontrolled pain/swelling
- erythema/red streaking
- night sweats/chills
- fever
- color/odor to drainage
- compliance with WB restrictions
Meniscal Tear
- loading + rotation injury
- medial>lateral
- debride vs repair
Meniscal Debridement ROM Limitations
-no limitations
Extension Limitations
-none
WB for Menisectomy
-WBAT
WB for Meniscal repairs
- NWB
- TTWB
ACL ROM Limitations
-ext none
OATS
- osteochondral autograft transfer system
- take plugs of chondrocytes from one area to put in another
- all in one procedure
ACI
- autologous chondrocyte implantation
- harvest cells and grow for 6 weeks and implant into pt
- less common
Post op ACI/OATS
- NWB 0-2 weeks
- TTWB (20-30#) 2-4 weeks
- PWB (50-60#) 4-6 weeks
- WBAT >6 weeks
TTWB
20-30 #
PWB
50-60#
Open Chain Post ACL
-may be limited 90-40*
Open Chain post cartilage defects
-open then progress to CKC
ACL Hamstring Grafts
- strength will return SLOWLY
- wait 4-6 weeks before loading to allow healing
More concern with:
contact vs non-contact injury
- non-contact injury
- worried about safety of return to sport
