-use fine filament form needle to treat trigger point/dysfunctional neuromuscular junction

Goal:
A
- to elicit a local twitch response in the involved muscle
- desensitize supersensitive structures and restore motion and function

A
-localized areas of deep tenderness within taut band of muscle

Causes of Trigger Points
A
- posture
- repetitive movments
- compressive forces
- trauma
- restriction of biomechanics of joint

goals achieved by
A
- releasing shortened muscles
- remove source of irritation
- promote healing (produce local inflammation)
- decrease spontaneous electrical activity of trigger point
- decrease pain associated with active trigger points

Contraindications
A
- inadequate knowledge
- pregnancy
- bleeding disorder
- high dose anti-coagulants
- immunosuppressed
- local infection
- umbilicus, external genetalia
- area over joint replacement, breast implant, pacemaker, spinal stimulator
- over ribs
- none for 1st 6 weeks post op

A
- T spine
- lung proximity
- sacral/coccyx/buttock
- neurovascular bundles assoc with femoral triangle, cubital fossa, supraclavicular triangle, pec major

Complications
A
- insertion pain
- vasovagal reaction
- forgotten needle
- stuck needle
- broken needle
- Mm soreness
- fatigue
- bruising
- infection
- pneumothorax
