what is portal hypertension?
A
- complication of cirrhosis
- increased pressure in portal venous system

what are the causes of portal hypertension?
A
- pre- hepatic
- intra hepatic
- post- hepatic

how does pre-hepatic LF cause portal hypertension?
A
thrombosis of portal vein

how does intra-hepatic LF cause portal hypertension?
A
- cirrhosis ( most common in the UK)
- schistosomiasis -( disease caused by parasitic worms)
- budd chiari syndrome ( hepatic vein obstruction)

how does post hepatic LF cause portal hypertension?
A
- RHS heart failure
- Constrictive pericarditis
- IVC obstruction

what are the symptoms of portal hypertension?
A
- usually asymptomatic
- present when oesophageal varices rupture

describe the pathology behind portal hypertension
A
- normal pressure 5-8 mmHg in portal vein
- cirrhosis= increased resistance to flow
- therefore splanchic dilation and compensatory increased CO
- fluid overload in portal vein
- results in collateral blood shunting to gastroesophageal veins
- become oesophageal varices at cardia and lower oesaphagus
