young people without known illness
What is a primary spontaenous pnuemothroax usually do to?
rupture of subpleural bulla
What is a secondary sponatenous pneumothorax due to?
patients with pre-existing pulmonary disease
What are causes for a pneumothorax?
- Spontaneous
- Chronic lung disease
- Infection
- Traumatic
- Carcinoma
- Connective tissue disorder
What is a tension pneumothorax?
medical emergency with immediate decompression
What causes tension pneumothorax?
trauma and mechanical ventilation
What are symptoms of a pneumothorax?
- Dyspnoea

What are symptoms of a tension pneumothorax?
- Distressed
- Rapid laboured respirations
- Cyanosis
- Profuse diaphoresis
- Tachycardia
What are the signs in a pneumothorax?
- Reduced expansion
- Hyper-resonance to percussion
- Diminished breath sounds on affected side
- Tension: trachea deviated away from affected side
What are possible DDx for pneumothorax?
- Acute asthma/COPD exacerbation
- PE
- MI
- Oesophageal perforation
- Giant bullae
What investigations are done for a pneumothorax?
- CXR
2. Blood test: baseline values
What would CXR show for pneumothorax?
- visible rim between lung margin and chest wall or surgical emphysema
- absence of lung marking between the lung margin and chest wall
What is the treatment for primary pneumothorax <2cm?
discharge, repeat CXR
What is the treatment for primary pneumothorax >2cm/SOB?
aspiration, if unsuccessful chest drain - analgesia (regular)
