Primary action for glucocorticoids
- Suppress leukocytes and inflammatory cytokines/chemokines
2. Topical inhibits histamine release and skin thinning
Side effects of glucocorticoids (Looks like Cushing Disease)
- Loss of muscle mass
- Muscle weakness
- Thinning of skin
- Impaired wound healing
- Osteoporosis
- Diabetogenesis: increased glucose levels-increase insulin release and lipogenesis
- GI-peptic ulcer
- Catabolic and anti-anabolic effects
Hydrocortisone, cortisone, prednisone
Short to Medium acting glucocorticoids
Dexamethasone
Long-acting glucocorticoids
Four uses for Glucocorticoids
- Allergic reactions
- Inflammatory bowel disease
- ARDS
- Dermatitis
Treatment of Addisons
Too little cortisol and aldosterone are produced. Use hydrocortisone daily
Ketoconazole
Inhibits adrenal steroid synthesis. Used to treat Cushings, osteoporosis, and Pagets
Metyrapone
Selective inhibitor of cortisol/corticosterone synthesis. Used to treat Cushings, osteoporosis, and Pagets
Cause of Paget Disease
- Cause not known, but may be triggered by viral infections
- Normal osteoclastic and osteoblastic remodeling activity is disrupted resulting in excessive bone removal followed by excessive bone formation leading to an irregular and weakened bone pattern and increased likelihood of fractures.
