What is the most common cause of complicated UTI and what is the most significant clinical presenting sign?
Indwelling catheter. Severe dysuria.
First line treatment for complicated cystitis, 2 drugs? Which one is not recommended?
Ciprofloxacin or levofloxacin. No go on moxifloxacin
What 2 drugs do we use against enterococcus UTI?
Ampicillin or amoxicillin
What do we use for complicated Mild pyelonephritis first line? 4 drugs and their order.
- Cetriaxone.
- Cipro or Levo
- Aztreonam
What is first line treatment for severe complicated pyelonephritis? What are the three combos?
Beta lactam and beta lactamase inhibitor
- Piper + tazobactam
- Cefto + tazobactam
- Ceftazadime + avibactam
What can you give first line for severe complicated pyelonephritis in place of the combo?
Carbapenem
Advantageous element of carbapenems and what is their MAO?
They are fairly resistant to beta lactamase and they inhibit transpeptidase.
What is the most common pathogen for prostatitis and what is the clinical symptom?
E Coli. Spiking fever.
What are the three drugs to treat Prostatitis?
TPM, Cipro, or levo
How do we manage post strep GN?
Loop diuretic, anti hypertensive and dialysis
How do we treat recurrent post strep GN?
First give penicillin G. Then a 1st generation cephalosporin. Then a 3rd generation cephalosporin. Then amoxicillin or clindamycin.
What is clindamycin very effective against and what is the adverse effect with clindamycin?
Anaerobes. C diff induced diarrhea and colitis
