Range of syndromes caused by atherosclerosis in lower limbs
Aetiology and risk factors of Peripheral vascular disease (acute and chronic limb)
Chronic-Most commonly caused by atherosclerosis
after a while, can cause tissue loss, hair loss, ulceration, thin skin
risk factors
all risk of artherosclerosis
-Smoke, fat, old, FHx, HTN, men etc
Epidiemology of Peripheral vascular disease (acute and chronic limb)
Increase with Age-25% of over 85% have it
5% of over 60
Signs and Sx of Peripheral vascular disease (acute and chronic limb)
CHRONIC:
Most commonly asymptomatic
Intermittent claudication, relieved with rest and worse as walking
pallor leg when elevated (someone’s sign)
Thigh/buttock pain
Critical limb-ABPI < 0.5
more bad-pain at rest, gangrene, can cause tissue loss, hair loss, ulceration, thin skin-risk of losing limb
Diminished absent limb pulse
cold extremities
reduced CRT
ACUTE-Thrombosis-6P’s-rest pain
Pain pallor pulselessness parasthesia, peringshingly cold paralysis
blood flow-severly impaired
Investigations of Peripheral vascular disease (acute and chronic limb)
Ankle brachial index-under 0.9 is intermittent claudication,
under 0.5 is critical limb
Managmenet of Peripheral vascular disease (acute and chronic limb)
Chronic -lifestyle (exercise, better diet, more statins)
Anything more severe-surgery (bypass)
Acute-Surgical thrombectomy
