peripheral nerves Flashcards

1
Q

30 yo woman– motor vehicle accident. slams her left shoulder into the steering wheel, unable to extend and suppinate the left arm, and has decreased sensation on the posterior aspect of the upper arm, forearm and hand. What nerve is likely injured? what motor injuries? what sensory loss?

A

radial– innervates posterior muscles of the arm and forearm.

  • loss of extension at the elbow, wrist and digits, weakened extension at the shoulder, and weakened supination
  • sensory loss on posterior arm, forearm, and hand

Q

median nerve- what does it innervate?

A

muscles of the ant. comp. of the forearm EXCEPT carpi ulnaris and half of the flexor digitorum profundus (it supplies the pronator teres, flexor carpi radialis, palmaris longus, flexor digitorum superficialis, flexor pollicis longus, pronator quadratus, abductor pollicis brevis, opponens pollicis, superficial head of the flexor pollicis brevis, the 1st and 2nd lumbricals, and half of the flexor digitorum profundus)

Q

injuries of the median nerve at the elbow and wrist cause clinical problems with respect to

A

the forearm, wrist and hand mainly with flexion and pronation. Weakened extension and weakened supination

Q

musculocutaneous nerve innervates…

injury to musculocutaneous nerve causes…

A

muscles of anterior arm

injury: loss of elbox flexion, weakness in supination and loss of sensation on lateral aspect of forearm

Q

ulnar nerve innervates… clinical problems arise at what level?

A

muscles of the forearm not innervated by median nerve, and all the muscles of the hand except those innervated by median (flexor carpi ulnaris, palmaris brevis, abductor digiti minimi, flexor digiti, minimi brevis, opponens digiti minimi, palmar and dorsal interossei, 3rd and 4th lumbricals, medial half of the flexor digitorum profundus). Clinical problems arise with injury at the elbow and wrist

Q

what peripheral nerve disease frequently follows viral ifxn, and has rapidly evolving limb weakness with symmetric hyporeflexia but normal sensation. (sensory symp can occur, but motor symp predom). the symptomes typically ascend the body —> may evolve into complete paralysis with resp failure. It can be an autoimmune disease and is correlated to demyelination…

A

Guillain-Barre syndrome

Q

is palm sensation affected by carpal tunnel syndrome- why or why not?

A

no, because the superficial palmar cutaneous branch of the median nerve passes superficial to carpal tunnel

16
Q

trans-synaptic degeneration occurs when…

A

a nucleus in the CNS degenerates in response to degeneration of it’s afferent pathway

17
Q

In patellar reflex arc, what are the afferent fibers that sense tension within the intrafusal fibers and detect muscle length?

A

spindle afferents

18
Q

hyporeflexia means it’s what type of problem?

A

LMN

19
Q

hyperreflexia means you have what type of problem?

A

UMN

20
Q

suprascapular nerve is what two nerve roots?

A

C5 and C6

21
Q

damage to the suprascapular nerve results in (2 things)…

A
  1. loss of shoulder abduction btwn 0-15 degrees (supraspinatus muscle)
  2. weakness of lat. rotation of shoulder (infraspinatus muscle)
22
Q

long thoracic nerve is what nerve roots?

A

C5, C6, C7

23
Q

lesions to the long thoracic nerve causes what 2 problems?

A
  1. loss of abduction of the arm above the horizon to above the head
  2. winged scapula- pt unable to hold the scapula against the posterior thoracic wall
24
Q

the long thoracic nerve (C5-C7) is often injured from…

A

radical mastectomy or a stab wound to the lateral chest (nerve lies on the superficial surface of serratus ant. muscle)

25
Q

a winged scapula occurs from inj to what nerve?

A

long thoracic nerve
