describe the pathophysiology of peptic ulcer disease
1 Depends on cause :
- Reduction in protective prostaglandins or
Increase in gastric acid secretions
2 Acidic contents of stomach/duodenum
break down mucosa
3 Pain varies with acid level affecting
area of ulcer
define peptic ulcer disease
A break in the epithelial cells which penetrate down to the muscularis mucosa of either the stomach or duodenum
what are the complications of peptic ulcer disease?
upper GI bleed
- gastric ( less common)
- duodenal ( more common)
where do gastric peptic ulcers occur?
mostly at lesser curve
how common are gastric peptic ulcers?
less common
what are the causes of both gastric and duodenal peptic ulcers?
- H pylori
- NSAIDS
- Zollinger Ellison syndrome
- increased gastric acid
- bile reflux
where do duodenal peptic ulcers occur?
mostly at D1/D2 ( duodenum 1/2) posterior wall
how common are duodenal peptic ulcers?
more common 2-3 x more
what are the symptoms of gastric peptic ulcers?
-Weight LOSS
-Nausea
-Heartburn
- epigastric pain- worse on eating
- better between meals + with antacids
what are the symptoms of duodenal peptic ulcers?
- weight GAIN
- nausea
- heartburn
- epigastric pain worse between meals
- better with food
how do you diagnose peptic ulcers?- if no red flags
if no red flags
- non invasive tests
— C Urea breath
—- stool antigen test
how do you diagnose peptic ulcers?- if red flags
if red flags
- endoscopy + biopsy —> will show brunners gland hypertrophy
how are peptic ulcers treated?
-stop NSAIDs
- If H pylori +ve
– triple therapy CAP antibiotics
- stop smoking
- potion pump inhibitors
