coeliac disease Flashcards
1
Q
define coeliac disease
A
- an autoimmune condition where exposure to gluten causes mucosal inflammation of the small bowel and malabsorption
2
Q
describe the pathophysiology behind coeliacs disease?
A
- ingest gluten- wheat, barley, rye
- A - gliadin is the toxic part in gluten
- it triggers immune system by binding to IgA to make antibodies- anti- ttg and anti - EMA
- Targets epithelial cells of small intestine causing:
- villous atrophy
- crypt hyperplasia
- intraepithelial lymphocytes
3
Q
what is a tropical sprue?
A
- malabsorption syndrome
- similar biopsy to coeliac
- treat with antibiotics
4
Q
what is the treatment for coeliacs?
A
- stop eating gluten
- vitamin + mineral supplements
5
Q
what are the symptoms for coeliacs disease?
A
- malabsorption
- dermatitis herpetiformis
- osteoporosis
- low ca2+
6
Q
what are the three types of symptoms of coeliacs disease?
A
- malabsorption
- dermatitis herpetiformis
- osteoporosis
7
Q
how is coeliacs diagnosed?
A
- serology - screening
- duodenal biopsy- DIAGNOSTIC
8
Q
what does serology show for coeliac disease ?
A
- anti- tTG
- increased IgA
- increased EMA
9
Q
what does duodenal biopsy show?
A
- crypt hyperplasia and villous atrophy and epithelial lymphocyte infiltration
10
Q
what are the malabsorption symptoms for coeliacs?
A
haematinic deficiency - Fe, B12, Folate
- anaemia
- steatorrhoea
- diarrhoea
- weight loss and failure to thrive
11
Q
what are the dermatitis herpetiformis symptoms of coeliacs?
A
- red raised patches on elbows, knees and buttocks caused by IgA deposition
12
Q
what are the risk factors for coeliacs?
A
- other autoimmune diseases- T1DM
- IgA deficiency
- breastfeeding
- rotavirus infection
- genetic- HLA DQ2 or HLA DQ8
