What is an example of Penicillinase Resistant PCN
Nafcillin
Nafcillin MOA, clinical application, avoidance, and SE
MOA: inhibits cell wall, bacteriocidal
USE: Gram Positives: MSSA, CONs, COPs
Avoid: enterococci or MRSA
SE: Acute interstitial nephritis, phlebitis, bone marrow suppression
Aminopenicillin (Ampicillin) MOA and use
MOA: Bacteriocidal, inhibits cell wall
USE: some gram negative coverage , Strep and Enterococcus, CNS coverage, clostridia, listeria, N. Meningitis
Aminopenicillin (Ampicillin) avoid use and side effects
Avoid use in staph
SE: Rash not a true allergy
Ampicillin/sulbactam MOA and clinical applications
MOA: Bactericidal, inhibits cells wall and beta lactamase
USE: Gram pos, gram neg, and anaerobes, H. Influenza
Ampicillin/sulbactam avoid use and SE
Avoid use in strep
SE: Eosinophilia, phlebitis, fever, rash
Antipseudomonal penicillin (Piperacillin/Tazobactam) MOA and use
MOA: Bactericidal,, inhibits cell wall, and beta lactamase
USE: Gram pos, gram neg, anaerobes, MDRO
Piperacillin/Tazobactam avoid use and SE
Avoid use in ESBL
SE: AKI if given with vanco, impaired platelet aggregation, and bone marrow suppression
