pediatric platelet disorders Flashcards
platelet count when primary hemostasis is impaired
below 75k
platelet count when spontaneous bleeding occurs
below 50k
platelet count when noticeable/clinically significant bleeding occurs
20k
platelet count when life threatening hemorrhage occurs
10k
What is the most common cause of isolated thrombocytopenia in otherwise well children
acute ITP
Sudden onset of bruising and bleeding in an OTHERWISE HEALTHY CHILD, and 50% after viral illness
acute ITP
what is the time frame the separates acute from chronic ITP
6 months
peripheral smear with ITP
- schistocytes
- +direct coombs
- Fibrin degradation products and fibrinogen
describe the treatment for ITP
- No treatment alter natural history
- Prednisone, IVIG, anti-D immunoglobulin can increase count
- focus on keeping count above 20
What 2 diseases cause consumption of platelets resulting from endothelial injury and vasculitis
HUS and TTP
Compare and contrast HUS and TTP
TTP: adults, thrombocytopenia and hemolytic anemia
HUS: children, E. coli, microangiopathic hemolytic anemia, thrombocytopenia, and acute renal failure
only curative treatment of chronic ITP
splenectomy
if pancytopenia and maybe no LAD or constituational symptoms
aplastic anemia
pancytopenia with bloody diarrhea and maybe edema
HUS
