Sob on exertion or rest.

Productive/non productive cough. Colour of phlegm

Anxiety symptoms.
Sob+tachycardia =PE. 
Sob due to CHF. 
Dib when lying flat (Orthopnea). 
Ankle oedema.

Cough, Colour go phlegm. Pain upon manipulation of pectoral.  Symmetry of chest,
