Patella Tendinopathy Flashcards
What is patella tendinopathy
-degeneration of patella tendon
What is reactive tendinopathy
- temporary localised thickening of tendon in response to acute overload or trauma
- tendon can revert to normal structure if overload is reduced or sufficient time between loading
What is tendon dysrepair?
- chronic overload
- increase collagen production and separation
- tendon matrix breakdown
- matrix disorganisation
- ingrowth of vessels and nervs
- swelling
- tendon degeneration
- tendon weakness
What is Degenerative tendinopathy?
- Large areas of disordered matrix
- largely irreversible matrix breakdown
- absence of tenocytes and collagen
- Neo-vascular infiltration ++
- structural failure
What are the symptoms of patella tendinopathy ?
- gradual onset of anterior knee pain (below knee cap)
- aggravated by jumping, hopping and bounding
- ache and stiffness post activity
what are the signs of patella tendinopathy?
- Tenderness of inferior pole of body of tendon
- tendon thickening
- poor VMO function
- quadriceps wasting
- calf weakness
- pain with resisted knee extension
name 3 predisposing factors of patella tendinopathy
- muscle tightness
- muscle weakness
- patellofemoral joint biomechanics
what are the 4 injury classifications of patella tnedionpathy?
Grade 1, 2, 3 and 4
What is grade 1 injury classification?
pain only after training
what is grade 2 injury classification?
pain before training but eases once warmed up
what is grade 3 injury joint classification?
pain during training which limits performance
what is grade 4 injury classification?
pain during everyday activities
what investigations would take place?
- Ultrasound
- MRI
Name 4 bits of constructive rehab you would give
- rest 8-12 weeks avoiding agrravating factors
- strengthening exercises
- soft tissue therapy
- taping
- correction of biomechanical errors
what program would you give for eccentric squats?
- 3 sets at 15 reps
- twice daily
- 12 weeks
