What are some of the advantages and dsiadvatnages of using mortality as an outcome?
Easily defined, but not always recorded acuratley, not a good way of assessing outcomes and quality of care
What are some of the advantages and disadavatages of using morbity as a measure of health?
Routinely collected, however the collection is not always reliable and acucrate, and tells us nothing about the paitnets experiences, and is not always easy to use in a evaluation
What are patient reported outcomes?
Attempt to classify well being from the paitents point of veiw,and these are measures of the quality of life of a paitent that come directly from paitents
Why would you used patient related outcomes?
Increase in the conditions where management is the aim rather than curing, biomedical tests are just part of the pictures, and needs to focus on the patients concerns, and need to pay attention to the iatrogenic effects of care
What are some of the challenges of using PROMS?
Minimising the time and cost of collection, achieving high rates of patient protection, providing an apporitate output to different audiences and reducing the misuse of proms, and expanding to other conditions such as long term conditions
What is health related quality of life?
In clinical medicine represents the functional effect of an illness and consequent therapy on a patient, as perceived by the patient
What are some of the dimensions of health realted quality of life?
Physical function, symptoms, global judgements of health, psychological wellebing, social wellbeing, cognitive functioning, personal constructs and satsification with care
How can you use qalative measures to assess health realted quality of life?
Can be very approiate, and give you access to parts that other measures can’t reach, and good for looking at the intia dimensions of health related quality of life however is ver resource hungry
What are some of the uses of qunatitive measures of health realted quality of life?
Relies on the use of questionaires known as published instruments, and should fufil certain criteria
What are the two most important features of a PROMS instrument?
Reliability- is the instrument accurate over time, and internally consistent
Validity- does the instrument actually measure what is in intended to
What are generic instruments?
Can be use in healthy populations including healthy people, and also have questions on social and emtional and physical functioning, pain and self care
What are some of use of specific instruments?
Evaluates a series of health dimensitons specific to a disease site or dimensiton
What are the advantages of using generic insturments?
Can be used for a brad range of health problems, can be used as a disease specific instruments, and enable comparision across treatment groups, and can be used to determine the unexpected and expected results of treatmetns
What are the disadvantages of using generic instruments?
They are inherantly less detailed, loss of relevance, and can be less senstive to changes that occur as a result of interactions
