Pain Flashcards
pain is classified into which two categories?
nociceptive pain: appropriate response to painful stimulus via intact nervous system
neuropathic pain: inappropriate response caused by dysfunction to nervous system
what are the symtoms of neuropathic pain?
shooting/burning sensations
tingling and numbness
allodynia
hyperalgesia
allodynia and hyeralgesia are both innapropriate responses to pain- how do they differ?
allodynia: pain in response to stimulus not usually painful i.e. light touch
hyperalgesia: inc in pain response w lowered pain threshold to stimulus that is usually painful i.e. pinprick
what are causes of neuropathic pain?
spinal cord injury
MS
stroke
diabetic neuropathy
trigeminal neuralgia
how is neuropathic pain managed?
amitriptyline
duloxetine
gabapentin
pregabalin
can standard drugs used in management of neuropathic pain be given for trigeminal neuralgia?
No- carbamazepine first line in trigeminal neuralgia
what class of drug is amitriptyline?
tricylcic antidepressant
inhibits neuronal reuptake of noradrenaline and serotonin
what is the MoA of duloxetine?
inhibits reuptake of serotonin and noradrenaline
which drug used in management of neuropathic pain primarily works by inhibiting high-voltage activated presynaptic Ca2+ channels?
gabapentin
pregabalin
(GABA analogue)
which is complex regional pain?
chronic condition cuasing neuropathic pain usually affecting the arm or leg
what can precipitate complex regional pain?
srugery or minor injury
how is complex regional pain managed?
physiotherapy
amitriptyline
neuropathic pain most commonly occurs due to lesions in which area of the brain?
thalamic lesions
central post stroke pain occurs in how many stroke patients?
10%
how does central post-stroke pain present?
rapidly improving hemiplegia w paraesthesia followed by allodynia w abnormal pain sensations
