Alendronate

What are the indications for bisphosphonate use?
Primary prevention of osteoporosis in post menopausal women and steroid therapy
In which patient group do bisphosphonates have a weaker evidence base?
> 80 yo- may require just vitamin D or calcium instead, or longer acting bisphosphonates
What may be prescribed with bisphosphonates?
Vitamin D and calcium
What are the side effects of bisphosphonates?
Mild GI sx Oesophageal ulceration/erosion Diffuse musculoskeletal pain Osteonecrosis of the maxilla (rare) Hypercalcaemia
What other medications are used to treat osteoporosis other than bisphosphonates?
HRT/raloxifene- CV risk, malignancy
Teriparatide
Denosumab- severe hypcalcaemia
