What are the 10 steps of fitting a URA?

A
  • ensure patient details match the supplied appliance
  • check appliance matches design
  • inspect with finger for any sharp areas
  • check integrity of wirework (ie damage, work-hardening)
  • insert into patient’s mouth, checking for trauma or blanching
  • check posterior retention (Adam’s)
  • check anterior retention
  • activate appliance
  • demonstrate insertion/removal to patient (get patient to show you)
  • book review 4-6 weeks

What are the advantages of URA?

A
  • tipping teeth
  • easily adapted for OB reduction
  • block movements
  • excellent anchorage
  • cheaper
  • shorter chairside
  • OH easier to maintain
  • non-destructive

A
  • less precise

What information should you give to the patient when delivering the URA? (10)

A
  • appliance will feel bulky, you will get used to it quickly
  • hypersalivation should subside in 24 hours
  • may impinge speech
  • initial discomfort is normal (device is working)
  • wear 24 hours including eating and sleeping
  • remove after every meal and clean with soft brush
  • remove and store in protective container when playing contact or active sports
  • avoid hard, sticky foods and be cautious of hot drinks
  • missing appointments or not wearing appliance lengthens treatment time significantly
  • emergency contact details

How can you tell if a patient has been wearing their URA?

A
  • ask
  • did they wear into the surgery
  • can they speak with it in
  • are they suffering from excess salivation
  • can they insert/remove easily
  • are there signs of wear on appliance
  • does the palate look like they’ve been wearing it ie erythema
  • has the tooth moved/is active competent passive
  • does the appliance still fit
