Popular in the 1950’s. Promised Pt-centered care. Also responsive to post WWII RN shortage. Each unit has teams composed of variously educated providers. Typically, aides, orderly, LPN, RN with an experienced RN as the team leader. Team leader is ultimately responsible for care of the Pts but delegated appropriate tasks to other team members.

Popular during Great Depression of the 1930’s. Allowed for care of greater number of pts. Head nurse assigned tasks of appropriate skill level to other personnel on the unit. Each provider was responsible for performing their single task on all pts in the unit. For example, one nurse gave all mess to all pts and and tech took all temperatures on all pts. Very efficient. But care was fragmented as no one person was responsible for planning care.

Current trend in some areas. Pt care is provided to a group of pts by an RN and an LPN or nursing assistant. The RN plans and directs care and delegates appropriate tasks to his partner at their skill level. Aka “care pairs.” Disadvantage is the difficulty of maintaining consistent partnerships.

Pattern of care popular in the 1980’s. One nurse was assigned the responsibility for the care of each pt from admission until d/c. This nurse was responsible for creating and updating the care plan, pt teaching and d/c planning. An associate nurse worked with pt on other shifts but she follows the care plan of the main nurse.
