Can an authorized member from one apparatus transport expired controlled substance drugs from another apparatus to EMS for exchange?
yes, the exchange must be documented on both 900.26 drug logs
This will cause a temporary increase in the maximum number of controlled drugs on one apparatus but is authorized to accommodate the transfer.
The EMS office will dispose of expired drugs how often?
quarterly
ensure the following before change of drug custody
- The security seal is intact and the number matches the 900.26
- The two Versed outside the lockbox are visually inspected
- The off going member has completed the date, seal#, drug count and signature.
- if everything is correct above, the oncoming member fills out the next line of the 900.26
In the event that no crew will be assigned to an ALS unit for longer than one shift, the following shall apply:
- Controlled substance coordinator shall be notified
- Versed in the kits should be placed in the lockbox
- Fill out the 900.26 after breaking the seal to put the Versed in
Who should be notified if the drug count can not be reconciled?
The company officer and the controlled substance coordinator. If a further audit/check fails to find the drugs, the station Captain, BC, C103, and on duty investigator will be notified.
In the vent of lost or stolen drugs who should be contacted?
Station officer notifies the Captain. BC, C103. On duty investigator and the controlled substance coordinator.
For lost or stolen drugs, an incident report shall be written on PF&R interoffice memo, signed and forwarded to who?

Who files the DEA form #106?
EMS office will file all DEA reports
