Expression of Regret
- As early as possible, patient and support person should receive an
expression of regret for any harm resulted from the adverse event
Aim of Open Disclosure
To promote clear and consistent approach by healthcare professionals to open communication with patients and their nominated support person following an adverse event.

3) What is being done to prevent it from happening again?

Acknowledgement
- Acknowledge to the patient and support person that an adverse
event has occurred.

Confidentiality
- Policies and procedures are to be developed by a dental practice
with full consideration of the patient’s, carer’s and staff’s privacy.
- These should also be in compliance with the relevant laws of privacy.

Recognition of Reasonable Expectation of Patient & Support Person
- Patient and support person may reasonably expect to be fully
informed of facts surrounding the adverse event and consequences
- They can also expect to be treated with empathy, respect and
consideration and provided with the appropriate support.

Openness & Timeliness of Communication
- Any information provided must be in an open and honest manner
- Note that the open disclosure is a fluid process and may involve
provision of ongoing information.

Staff Support
- Dental practices should create an environment where staff is
encouraged to recognise & report adverse events and is supported
through the open disclosure process.

Integrated Risk Management & Systems Improvement
- Investigate the adverse event and focus to manage the associated
risks and improve system of care
- System and improvements should be constantly reviewed for their
effectiveness

Good Governess
- When system of care is improved base on investigation and
management of risks, the practitioner in charge must oversee and
enforce the changes to the system to prevent recurrence.

2) Act immediately to rectify the problem (including seeking out
necessary help
3) Expressing regret

happened including short and long term consequences
5) Acknowledge the patients distress and provide appropriate support
6) Comply with relevant policies, procedures and reporting
requirements subject to advice of a professional indemnity insurer

7) Report adverse events to the relevant authority
8) Ensure patients and clients have access to information about the
processes to make a complaint
9) State or agree that another health care professional is liable for the harm caused to the patient
