Immediate Actions:
1. Prior to reaching SP Temp or RPV Press limits, SCRAM the reactor.
2. If three or more SRVs open in Mode 1 or 2, SCRAM the reactor.
3. Evacuate Containment
4. Reduce power with Recirc flow to ≤ 96%.
5. When power is ≤ 96%, perform the following:
- Verify Rx Pressure < 1033 psig
- Attempt to close the SRV- place both switches in off.

If feed flow decreases to < 3.43 mlbm/hr for > 15 seconds, Recirc pumps will downshift to slow.
At vessel level 3 (178”):

 Recirc pumps downshift
 RHR SDC isolates.
At vessel level 8 (219”):
 RFPTs & MFP trip,
 Main Turbine/Generator Trip
 If in Run, Reactor Scrams
The Following cause a shift from 3 Element to 1 Element control:
 Loss of a valid steam flow signal
 Loss of a valid feed pump suction flow signal
 Loss of a valid feed pump recirc flow signal

Immediate Actions:
1. If flow malfunction occured, perform the following:

A. Transfer affected RFPT to manual dial

 And, if necessary,

B. Transfer MFP MAN/AUTO STATION to Manual
  1. If feed pump failure occured, start feed pumps as necessary to match steam flow and feed flow.

Entry Conditions
- Operation in MANUAL SCRAM REQUIRED region of the Power to Flow map when OPRMs Inoperable
- Operation in the CONTROLLED ENTRY/IMMEDIATE EXIT region that is NOT in accordance with an approved instruction when the OPRMs are inoperable
- Operation in the IMMEDIATE EXIT region of the Power to Flow map when OPRMs are operable
- FMEOD > 1.000
- Unplanned changes in reactor power
- FCV malfunction
- Recirc Pump trip
- Unintentional Recirc Pump downshift
- Recirc loop jet pump flow mismatch exceeding TS limits

Immediate Actions:
- If any of the following conditions exist, SCRAM the reactor:
a. More than one control rod inadvertently scrammed.
b. Unexplained power rise which cannot be quickly terminated.
c. Entry into manual scram region when OPRMs inoperable.
d. No recirc pump flow in Mode 1 or 2.
e. Power oscillations are observed.
- If flow cannot be controlled, arm and depress HPU S/D switch
- If OPRMs are operable, and entry into immediate exit region, insert cram rods IAW FTI-B02 to approx 35% power.

Evacuation of Control Room

A

Entry Conditions
Any unsafe situation such as fire, toxic gas, high airborne activity, or unforeseen emergency which requires evacuation of the Control Room. The Unit Supervisor shall have evaluated the possibility of remaining in the Control Room with breathing apparatus and shall evacuate the Control Room only if necessary.

Control Room HVAC trains shift to Emergency Recirc

Immediate Actions
1. Scram the reactor
2. Verify Mode Sw locked in S/D
3. Prior to evacuating control room, perform as many of the following as possible:
 Verify control rods are inserted
 Verify thermal power decreasing
 Depress TURBINE TRIP
 Place Div 3 DG DIESEL CONTROL XFER SW to Local
 Initiate subfloor CO2 if necessary.
 Evacuate the Control Room

- INSTR VOLUME DRAIN VLV CLOSED are on.

Automatic Actions
- All withdrawn control rods are fully inserted into the core.
- SDV Vent valves close.
- SDV Drain valves close
- MSIV Closure on low steam pressure of 807# when MODE SW in RUN.
- Setpoint Setdown fires if below 178”
- Recirc pump downshift and FCVs to manual if any downshift signals occur.
- Recric Pump trips from EOC RPT or ATWS RPT

If required to prevent an ADS initiation, place affected channel ADS LOGIC INHIBIT switch in INHIBIT

FPCC Pumps trip when FPCC surge tank level < 8.5”

Immediate Actions
Note: The immediate actions of this ONI are associated with loss of DHR due to loss of fuel storage pool level, specifically when fuel is being.
1. Make announcement to evacuate all unnecessary personnel - Repeat announcement until access is controlled and all unnecessary personnel are evacuated.
2. Suspend core alts and ensure all fuel is in safe condition.
