What is OA?
Cartilage loss and inflammation of joints
- causes the bones to rub against bone = pain
- associated with ageing, wear and tear, old injuries
Components of the synovial joints
- Aggrecan
- Hyaluronan (HA)
And what happens when we age
-acts as a sponge in the cartilage, allows for weight
bearing activity, as it compresses water is squeezed out
also contains chrondratin sulfate and keratin sulfate
-backbone for Aggrecan
- aggrecan gets shorter - less water absorbing ability
- change in matrix turnover: cartilage softens, cracks and falls away
Fish oil
There isnt much evidence at all the support this
-it is meant to promote EPA thus less AA and less inflammatory response
Glucosamine
this makes up the hyaluronan (component in cartilage)
in theroy prevents the degrdation of cartilage
DO NOT give with shellfish allergy
Chrondroitin
Makes up aggrecan (component in cartilage), made from shark cartilage
What is the difference between Ibuprofen and Naproxen?
Ibuprofen - lasts about 2 hrs -metabolised by liver first, then excreted Naproxen -lasts about 15hrs excreted by kidney
Interactions with NSAIDs
Triply whammp - Nsaid, Diuretic, ace
Avoid in HF - becuase it can cause fluid retention, thus increase Blood pressure
-gastric ulcers - avoid if there is a history here
-renal impairment - increase conc
pregnancy - delays labour
-increase risk of bleeding
Intra articular corticosteriods
- Induces Annexin-1
- stoping PG, - Stops neutrophils
- increasing healing and repair
Knee only
IA hyaluronic acid
- oiling the joint - libricant
2. Prevents MAP kinases from degading chondrocytes
what is aggrecan made of ?
Chrondrotin sulfate and keratin sulfate and proteins all atttached to a hyaluranan central backbone
