Nutrition Support Flashcards
1
Q
events that are bypassed during IV feeding
A
- stimulation of mucosal cells by luminal nutrients
- stimulation of GI hormone response
- direct transport of nutrients to the liver (first pass)
- packaging of lipids in chlyomicrons
2
Q
definition of enteral feeding
A
- delivery of nutrients to the GI tract through a feeding tube into the stomach or the small intestine
3
Q
definition of parental feeding
A
- delivery of nutrients into the vascular system
- requires nutrients miscible with blood and components that are not immunogenic
4
Q
indicator of muscle mass
A
- urinary creatinine
5
Q
effect of fever on metabolic rate
A
- metabolic rate increases by approximately 10% for every degree above 37C
6
Q
pathophysiology of refeeding syndrome
A
- triggered by aggressive oral, enteral, or parental carb feeding following a period of nutritional deprivation
- sudden influx of glucose into cells adapted to low glucose metabolism
- jump in glucose metabolism strains intracellular and extracellular reserves of phosphorus, magnesium, and potassium
- the shift from extracellular to intracellular can abruptly lower blood concentrations of these minerals
- severe mineral and fluid imbalances that occur with refeeding can lead to cardiac arrest, neuromuscular complications, and respiratory dysfunction
7
Q
short bowel syndrome
A
- initially there is malabsorption due to a decrease in the absorptive surface and due to decreased transit time
- parental nutrition support is necessary until the intestines are able to adapt and absorb the adequate amounts of nutrients and fluids
8
Q
complications of long-term TPN
A
- catheter related infection
- liver disease
- metabolic bone disease
- micronutrient deficiencies
9
Q
complications of enteral feeding
A
- reflux of stomach contents into the lungs
