Routine assessment of nutritional and developmental milestones; anticipatory guidance about common sources of lead exposure; follow-up blood lead testing at recommended intervals based on child’s age
Blood Lead Level 5-9ug/dL (3)
- Routine assessment of nutritional and developmental milestones; environmental assessment*** including detailed history to identify potential sources of lead exposure
- Nutritional counseling related to calcium and iron intake
- Follow-up blood lead testing in 3 months, once it is declining follow up in 6-9 months
Blood Lead Level 10-19ug/dL (3)
- Routine assessment of nutritional and developmental milestones; environmental investigation*** including home visit to identify potential sources of lead exposure
- Nutritional counseling related to calcium and iron intake; consider lab work to assess iron status
- Confirm blood test level in 1 week-1 month; Follow-up blood lead monitoring in 1-3 months, once it is declining at 3-6 months
Blood Lead Level 20-44ug/dL (5)
- Complete history and physical exam with neurodevelopmental assessment
- Environmental investigation and lead hazard reduction
- Lab work: iron status and hgb and hct
- Abdominal X-ray (with bowel decontamination if indicated)
5 Confirm blood test level in 1 week-1 month; Follow-up blood lead monitoring in 2 weeks-1 month
Blood Lead Level 45-69ug/dL (6)
- Complete history and physical exam with neurodevelopmental assessment
- Environmental investigation and lead hazard reduction
- Lab work: iron status and hgb and hct
- Abdominal X-ray (with bowel decontamination if indicated)
- Oral chelation therapy; consider hospitalization if lead-safe environment cannot be assured
- Confirm blood lead level in 24-48 hours; Follow-up blood lead monitoring ASAP
Blood Lead Level >/=70ug/dL (2)
- Hospitalize and commence chelation therapy in conjunction with consultation with a medical toxicologist or a pediatric environmental health specialist
- Confirm level ASAP; Proceed with additional actions according to interventions for BLLs between 45-69ug/dL
Blood level of 5-9ug/dL time to confirmation testing and later follow up testing after BLL declines
Time to confirmation testing: 1-3 months
Later follow-up testing after BLL declines: 6-9 months
Blood level of 10-44ug/dL time to confirmation testing
Time to confirmation testing: 1 week-1 month
Later follow-up testing after BLL declines: 3-6 months
Blood level of 45-59ug/dL time to confirmation testing
Time to confirmation testing: 48 hours
Later follow-up testing after BLL declines: 1-3 months
Blood level of 60-69ug/dL time to confirmation testing
Time to confirmation testing: 24 hours
Later follow-up testing after BLL declines: 1 month
Blood level of >/=70ug/dL time to confirmation testing
Time to confirmation testing: Urgently as emergency test
Later follow-up testing after BLL declines: ASAP
