Subjective
Cues based on the senses of the nurse or other member of the health team

Secondary data
Identify the client’s health status. Make the nursing diagnosis
Diagnosis
Determine the goals and outcomes
Planning
Nursing process: doing , delegating and documenting
Implementation
Nursing process: judging whether the plan worked.
