Aircraft Accident
occurrence associated with the operation of an aircraft between the time any person boards the aircraft with the intention of flight and every one disembarks and in which any person suffers death or serious injury, or the aircraft recieves substantial damage
Fatal injury
Injury which results in death within 30 days of the accident
Incident
Occurrence other than an accident which affects or could affect the safety of operations
Serious injury
Hospitalization for more than 48 hrs within 7 days
Fracture of any bone, except fingers, toes or nose
Severe hemorrhages, nerve, muscle or tendon damage
Involves any internal organ
Second-or Third-degree burns, or any burn affecting more than 5% of the body
Substantial damage
Damage or failure which adversely affects the structural strength, performance, or flight characteristics and which would normally require major repair or replacement of the affected component
Immediate notification is required when?
Flight control system malfunction or failure
Inability of any required crew member to perform normal flight duties
In-flight fire
Aircraft collision in flight
Damage to property in excess of $25,000
Release of all or portion of a propeller blade, not caused by ground contact
Loss of 50% or more of electronic flight display
Aircraft overdue and believed to be involved in an accident
